This men's engravable medical alert dog tag is MADE-TO-ORDER in solid 14k yellow gold and features a textured, Star-of-Life medical alert symbol on the front.
This dog tag has a polished finish, with a flat edge and measures 1 1/2 inch (L) x 7/8 inch (W) with an 0.40 gauge thickness (1mm)
This dog tag pendant can be worn on most chains up to a thickness of 3mm.
The front and back can be engraved with important medical emergency information (scroll down for engraving options and instructions).

1. Tag Front Engraving
Engrave your most important identification information here. Name, primary medical condition and In Case of Emergency (ICE) contact number.
2. Tag Back Engraving
Engrave using upper case lettering for clarity and legibility. List restricted medications, medication allergies, chronic medical conditions, blood type and recent surgeries.
3. Tag Back Engraving
List any food allergies, and if you carry an EpiPen. Also detail any other medical conditions and any other ICE numbers such as a Doctor or guardian.
Step 1. Select Number of Lines
From the drop-down menu, select Text Engraving with the number of lines you want engraved.
Step 2. Select Font Style
Select your preferred engraving font style from the drop-down menu. (View Text Engraving Fonts)
Step 3. Type In Your Text
In the line fields that appear, type in your text as you would like it engraved.
